Intake Fields (CSU cover sheet)

  • Date of application
  • Student full name, email address, mailing address, and telephone number
  • Application year (current scholarship cycle)
  • Request type: Initial first year or Renewal for 2nd year
  • AGCRA active membership number
  • Current or planned CSU enrollment details
  • Date/projected date of graduation
  • Current academic year or credit-hour progress
  • Payee destination for award disbursement

  • CSU Monje-Murphy cover sheet
  • Membership verification
  • Proof of CSU acceptance/admission or current enrollment
  • Most recent transcript
  • Personal narrative
  • Any program-specific supporting documents requested by the MOI
